Helpful tips for your upcoming appointment

Determine whether we are participating providers with your health plan. (see accepted insurances list). If we are not billing insurance, payment in full is expected on the day of your visit.

New patients: Please print and complete the new patient paperwork. (See below)

Please bring a list of all the prescription medications that you take.

Bring current glasses, contact lenses, and contact prescription or box with you on the day of your exam.

Insurance co-pays and non-covered services are due at the time of service. We accept Cash, Personal Check, Visa, Master Card and Discover credit cards.

Please arrive 15 minutes early for your first visit to allow time for registration. In order to bill your insurance we must have your current insurance information. When you arrive, you will need to present your insurance card(s) and a form of photo identification.

We ask that you print out and complete the following form:
